    Realizing that employees need protection for themselves and their families against financial hardship of the high cost of medical care, MGA Technologies offers health benefits that provide coverage for hospital, surgical and other medical care. These benefits make up an important part of your total compensation.

    Eligibility
    All salaried employees of MGA Technologies are eligible to participate in the health benefits plan. The coverage begins on the first day of the month following ninety days of employment.

    Enrollment
    These forms can be obtained from the administrative office.
    MGA Technologies pays a large portion of the health insurance premium for employees but the employee is also responsible for a portion of the premium. Information regarding the costs and provisions of the plans will be distributed to you at orientation.

    How the Plan Works
    The company reserves the right to make changes to the benefits plans at any time. When the network of providers is utilized there is a copayment for doctor visits for preventive and specialist care. In-patient and out-patient hospital care is covered when using network facilities. There is a copayment for use of the hospital emergency room. Home care is also a prescribed covered service as is in-patient and out-patient mental health, substance abuse and other healthcare services. Prescription card is issued, $35 for non-preferred, $20 for name brand, $8 for generic.

    Various wellness programs may also be offered through the healthcare carrier. Please refer to your booklets for details of the Plans.

    Termination of Coverage
    (COBRA)Coverage will terminate on the next premium due date following the date on which the Subscriber or Subscriber’s dependent ceased to meet the eligibility requirements. Coverage will also cease in the event that the Subscriber or Subscriber’s dependent fails to make the required co-payments.Coverage will terminate in the event a Member acts fraudulently or makes material misrepresentation in applying for or obtaining coverage under this contract or misuses the Identification Card by allowing a non-member to obtain benefits through use of the Member’s coverage.If coverage terminates due to the termination of the Subscriber’s employment other than for reason of gross misconduct, or due to a reduction in hours, the employee or employee’s dependent may elect to continue coverage under COBRA for 18 months. Members may elect continuation of coverage for 36 months under COBRA if the member’s coverage terminates due to:· a divorce or legal separation

    Subscriber’s death
    The subscriber becomes entitled under Medicare The dependent ceases to be a dependent child If you choose this continuation coverage you are entitled to identical coverage provided for the full-time employees at 100% of the monthly premium cost. You must apply for coverage within 30 days of the end of your coverage.

    If you do not choose continuation of coverage upon termination, your continuation of coverage under COBRA ends.



    Life Insurance
    MGA Technologies provides a core term Life Insurance/AD&D amount of $50,000 through Guardian. In addition, the employee has the ability to purchase supplemental amounts of Life and AD&D through a separate carrier, for him/her self in addition to his/her spouse and dependent children.



    Dental Plan
    MGA Technologies provides a dental plan to employees and their families.

    Eligibility: All salaried employees are eligible to receive dental coverage.

    Coverage: Preventative Services at 100%
  • Basic Services at 100%
  • $50 annual deductible per covered person
  • Major Services - 50% after 12 months of coverage are completed.
  • Annual benefit maximum per covered person is $1,500
  • Services in excess of $200 must be pre-approved by the Guardian Insurance Company.

    401K
    MGA Technologies has elected OppenheimerFunds as our 401K Retirement Plan investment company.

    OppenheimerFunds offers MGA over a dozen funds enabling our employees to customize long term financial planning. As shareholders, we are able to freely switch investments within the OppenheimerFunds family as our goals change over time.

    Some of the convenience services offered are personalized account assistance, 24 hour automated account transactions and fund information, telephone access to investments, and online access to valuable information pertaining to individual accounts.

    This 401K plan offers you the opportunity to save from 1% to 15% of your annual compensation, to a maximum of $11,000.00 per year (indexed annually). You pay no taxes on your investment until you withdraw it at age 59 ½ or later. This benefit is available to employees on the first day of employment.

    Sick Leave
    Absenteeism and/or tardiness are considered disruptive to the efficient functioning of our company. Consideration will be exercised towards employees who are absent or late for legitimate and serious reasons, and who have reported their absence or lateness to their assigned manager. However, unexcused and excessive absenteeism is cause for disciplinary action, and/or dismissal.




    Holiday

    It is the policy of MGA Technologies to recognize certain designated days throughout the year as paid holidays. Under normal circumstances, the company will observe seven (7) paid holidays during the calendar year. Employees will receive holiday pay equal to the number of hours regularly scheduled to work. Compensation will be paid at his/her base rate of pay exclusive of premiums, etc. Employees are eligible to receive holiday pay upon completion of ninety (90) days of successful employment.

    The scheduled paid holidays MGA Technologies provides are: New Year’s Day Memorial Day Independence Day Labor Day Thanksgiving Day Day After Thanksgiving Christmas Day If a holiday falls on a Sunday. the office will be closed the following Monday. If a holiday falls on a Saturday, the holiday will be observed the previous Friday. Employees who work on any of the seven scheduled holidays will be paid for working the holiday or will receive another day off for the holiday. A holiday, which occurs during an employee’s vacation, will be considered a holiday, not annual leave. The employee may not be paid for more than one benefit for the same hours.
